reporting a problem

When reporting faults please answer the following questions, we need this information to sort the problem out quicker.

  1. login name of the user affected
  2. this form completed by
  3. which machine or machines are affected
  4. when did the problem occur - date & time
  5. which program was being used - note the finder is a program
  6. which other programs (if any) were also running
  7. has the machine been restarted
  8. can the problem be recreated - does it still happen
  9. what are the symptoms of the problem
